Washington, D.C  - National Conference on Eating Disorder Legislation and State Advocacy Summit on June 29-July 1, 2005. The Eating Disorders Coalition for Research, Policy, and Action is about to take a historic step forward. At the end of June, during Eating Disorders Advocacy Week, participants of a conference in Washington, D.C., will write a "dream bill" for eating disorders and will present it to Congress. Evanston, IL Workshop - The Emotional Connection to Food. The relationship we have with food is a crucial link to our quality of life, positive body image, and long-term health. However, many women and men have adversarial relationships with food and their bodies. This adversarial relationship can provide a fertile ground for eating disorders and obsessive relationships with food. Precious time and energy is spent on trying diet after diet, experimenting with magic pills, which ultimately results in damaged self-esteem and negative body image. In this workshop, techniques to improve the connection between the mind and the body’s natural messages of hunger will be discussed. The presenters will explore how food is used to handle stress, sadness, and emptiness. Comorbidity Issues and the Treatment Process: Epidemiological research conducted throughout the world indicates that individuals who suffer from one mental health problem frequently meet the diagnostic criteria for one or more additional disorders. Since comorbidity is so common, it is imperative for clinicians to broaden their diagnostic and therapeutic skills so that they can respond effectively when confronted with the unique challenges that comorbid clients present. Clients with comorbid problems typically require longer or more complex treatment, or multiple episodes of treatment.  Consequently, treatment may be affected by the client’s financial limitations, or restrictions imposed by managed care.   These circumstances require therapists to be flexible in how they structure treatment. Clients with comorbid difficulties may acknowledge one diagnosis, but be in “precontemplation” about other diagnoses or problems.  As a result, the presenting complaint may not be the most effective focus of treatment (e.g., when an individual with a significant substance abuse problem seeks treatment for an eating disorder).  The therapist should learn to use motivational enhancement strategies to enable these clients to acknowledge the impact of comorbid problems. However, in many instances, the therapist will need to stick with the client through successive (and possibly less than successful) treatment contacts, while attempting to shift the focus of therapy towards the unacknowledged difficulties which need to be addressed in order for recovery to proceed more effectively. Since depression is the most frequent comorbid problem, it is vital for therapists who specialize in other areas to have a good working knowledge of affective disorders. Likewise, therapists who specialize in depression need to develop a working knowledge of the problems that typically overlap with the affective disorders, e.g., anxiety, eating disorders, and the addictions. The high prevalence of depression among all the other mental health disorders also underscores the importance of having clients screened for medical illness and medication side effects, since mood problems can arise from such difficulties. Moreover, therapists need to have a reasonable understanding of the medications available for mood disorders, and the benefits and risks associated with them. Lastly, although it is impossible to limit one’s practice only to “pure” cases of one’s area of specialization, it is important to acknowledge the disorders that are outside of one’s area of competence, and steer the client towards the appropriate sources of help. Those with histories of childhood abuse or trauma often present comorbid problems. These clients demand that therapists develop a working model of the complex sequelae of childhood abuse, and the skills to utilize a client’s history constructively in treatment. To be successful, treatment of trauma survivors typically will require a dual focus on both symptoms and underlying beliefs or issues. Focusing solely on symptoms may not be successful. Therapists also should not automatically assume that a client with comorbid problems always has a history of childhood trauma.

Research on Eating Disorders:

Personality characteristics of women before and after recovery from an eating disorder. Klump KL, Strober M, Bulik CM, Thornton L, Johnson C, Devlin B, Fichter MM, Halmi KA, Kaplan AS, Woodside DB, Crow S, Mitchell J, Rotondo A, Keel PK, Berrettini WH, Plotnicov K, Pollice C, Lilenfeld LR, Kaye WH. Department of Psychology, Michigan State University, East Lansing, MI 48824-1116, USA. BACKGROUND: Previous studies of personality characteristics in women with eating disorders primarily have focused on women who are acutely ill. This study compares personality characteristics among women who are ill with eating disorders, recovered from eating disorders, and those without eating or other Axis I disorder pathology. METHOD: Female participants were assessed for personality characteristics using the Temperament and Character Inventory (TCI): 122 with anorexia nervosa (AN; 77 ill, 45 recovered), 279 with bulimia nervosa (BN; 194 ill, 85 recovered), 267 with lifetime histories of both anorexia and bulimia nervosa (AN + BN; 194 ill, 73 recovered), 63 with eating disorder not otherwise specified (EDNOS; 31 ill, 32 recovered), and 507 without eating or Axis I disorder pathology. RESULTS: Women ill with all types of eating disorders exhibited several TCI score differences from control women, particularly in the areas of novelty-seeking, harm avoidance, self-directedness, and cooperativeness. Interestingly, women recovered from eating disorders reported higher levels of harm avoidance and lower self-directedness and cooperativeness scores than did normal control women. CONCLUSIONS: Women with eating disorders in both the ill and recovered state show higher levels of harm avoidance and lower self-directedness and cooperativeness scores than normal control women. Although findings suggest that disturbances may be trait-related and contribute to the disorders' pathogenesis, additional research with more representative community controls, rather than our pre-screened, normal controls, is needed to confirm these impressions.

 

Early separation anxiety and adult attachment style in women with eating disorders. Troisi A, Massaroni P, Cuzzolaro M. Department of Neurosciences, University of Rome Tor Vergata, Rome, Italy. OBJECTIVE: To ascertain whether women with eating disorders have a higher frequency of separation anxiety symptoms in childhood, and a higher prevalence of insecure styles of adult attachment, compared with healthy women. METHODS: The Separation Anxiety Symptom Inventory (SASI) and the Attachment Style Questionnaire (ASQ) were administered to 78 women with eating disorders and 64 healthy women. RESULTS: Compared with control women, women with eating disorders reported more severe symptoms of separation anxiety during childhood, and scored higher on the ASQ scales, reflecting insecure styles of adult attachment. Early separation anxiety and insecure attachment were not correlated with age of onset or illness duration. The diagnostic subgroup was not associated with a specific style of insecure attachment: compared with control women, both anorexic and bulimic women scored higher on the ASQ scales reflecting anxious attachment, but not on the scales reflecting avoidant attachment. CONCLUSIONS: The results confirm the link between eating disorders and insecure attachment that has been found in previous studies, and extend it to childhood symptoms of separation anxiety.

 

Research Study - Osteoporosis in Anorexia Nervosa: The Neuroendocrine Unit of Massachusetts General Hospital is conducting research studies on bone loss in anorexia nervosa. Their screening study is for men and women age 12-50 with anorexia nervosa. They are investigating causes of osteopenia (or bone loss) in the spine, hip, wrist and total body. Research Study - Massachusetts General Hospital Study of Therapy for Bone Loss in Anorexia Nervosa: The Neuroendocrine Unit of Massachusetts General Hospital is conducting research studies on anorexia-induced bone loss. Their treatment study is for women aged 18-45 with anorexia nervosa and absent or irregular menstrual periods. They are investigating the combined use of a natural hormone and a medication that is effective for bone loss in postmenopausal women as a novel treatment for the bone loss seen in women with anorexia nervosa. They hope that the combination of these two investigational medications will help rebuild depleted bone and prevent further bone breakdown in women with anorexia nervosa. The study has 4 outpatient visits and 2 inpatient visits over 12 months. A stipend of up to $675 is awarded throughout the course of the study. The National Institute of Mental Health - The NIMH is sponsoring a study assessing appetite and reward in eating disorders. Researchers need women who are 18 to 45 years old who are recovered from anorexia and/or bulimia nervosa and who are not taking psychoactive medications. Studies are taking place at the University of Pittsburgh Medical Center. The study involves the completion of interviews and questionnaires and subjects undergo a non-invasive magnetic resonance imaging of the brain. There is no X-ray exposure. The National Institute of Mental Health - Research on Genes and Eating Disorders: The National Institute of Mental Health is sponsoring a multicenter, international study seeking to determine whether a gene or genes might predispose individuals to develop anorexia nervosa. They need families with at least two relatives (e.g., sisters, brothers, cousins, aunts, uncles, grandparents) who have or had anorexia nervosa, and who would be willing to participate. Arizona Treatment Center News: Announcing the Emotional Eating Program at Remuda Ranch: Women and girls suffering with eating disorders have different issues, different needs. This is why Remuda Ranch provides intensive treatment in three separate and distinct facilities in Wickenburg, Arizona. Their adult program addresses the unique needs of women age 18 and older; their adolescent program treats teenage girls age 13 to 17; while their children’s program focuses on helping young girls, age 12 and younger. Agoura Hills, CA - Eating Disorder Intensive Outpatient Program (EDIOP). Carolyn Costin will be opening a new Eating Disorder Intensive Outpatient Program (EDIOP) in Agoura Hills, CA. This program is for females struggling with an eating disorder who need support in addition to their individual therapy but do not need full day treatment or inpatient care. The program will meet three nights a week and will include a supervised meal. Groups will be co-facilitated by therapists specialized in the treatment of eating disorders. The program will utilize a multi-disciplinary approach including cognitive-behavioral, dialectal-behavioral, psycho-educational, solution-focused, art therapy, attachment theory and process group modalities. In addition, the group facilitators will introduce and practice specific relaxation techniques useful in reducing anxiety and processing emotion. Colorado Treatment Center News: The Eating Disorder Center of Denver offers a full spectrum of care from partial hospitalization through outpatient to males and females age 16 and older. The Partial Hospitalization Program operates 11 hours per day, 7 days per week and includes supervised meals and snacks. The Evening Intensive Outpatient Program operates 4 hours per evening, three days per week. Most of the Eating Disorder Center of Denver's program components are available on an outpatient basis. Services include individual and family/couples therapy; group therapy including DBT; aftercare; psychiatric evaluation; therapy and medication monitoring and nutritional counseling. Eating Disorder Center of Denver’s Outpatient Groups: To register call 720-889-4232:

1. Aftercare Group: This group meets Monday evenings from 7:00-8:30pm and Tuesday evenings 5:30-7:00pm. The group is an open group consisting of individuals in recovery who have completed an intense eating disorders program at EDC-D or other national programs. Cost: $50 per group.

2. DBT Skills Training: The adult group meets Monday evenings 5:00-6:30pm; the adolescent group meets Tuesday evenings from 4:00-5:30pm. The group consists of didactic and experiential education in four different skill areas: core mindfulness, interpersonal effectiveness, emotion regulation, and distress tolerance. Cost: $75 per group.

Denver, CO - Minding the Body Support Group: Minding the Body helps individuals struggling to establish positive lifestyles after gastric bypass surgery or other weight loss programs. Weekly groups provide participants with the tools, support, and skills to maintain positive weight goals, deal with issues that may interfere with weight loss maintenance, and interact with others in various stages of the weight maintenance process. Florida Treatment Center News - "Reality Based Treatment for Eating Disorders." Milestones Eating Disorders Program offers residential treatment for anorexia, bulimia, compulsive overeating, and related disorders. The program is unique in so far as offering a supportive and structured environment while providing real world experiences such as preparing meals, grocery shopping, and participating in an array of activities simulating one's at home setting. Participants live within an apartment setting and are afforded opportunities to attend a variety of evening support groups while attending group and individual therapies during the day. The program is part of a facility, which is JCAHO accredited (Joint Commission for Accreditation of Healthcare Organizations) and licensed by the State of Florida to provide residential treatment as well as intensive outpatient services. The overall philosophy of Milestones is one of providing the treatment within a less restrictive setting than the traditional hospital environment and providing people with ample opportunities to put into practice those skills necessary to maintain their recovery when they return home. Milestones is intended for those who are not in need of a hospital setting and are able to demonstrate a sincere desire to recover from their eating disorder. Birmingham, AL – Free Eating Disorder Support Group: Sponsored by ANAD (National Association of Anorexia Nervosa and Associated Disorders). This group meets every Tuesday from 6:30-8:00pm at Vestavia Hills United Methodist Church Main Building Room on 318 Kentucky Avenue, Vestavia Hills, AL. This group is designed to provide: support and recovery information for those living with any eating disorder; resources and hope to the community regarding eating disorders and recovery; a common ground for sharing experiences and honest feedback in a safe environment. Glenside, PA Support Group: Women's Group on Tuesdays from 6:00- 7:30pm. This group encourages women to explore thoughts and feelings in a supportive and safe environment. Exploration of family, school and peer relationships will be discussed with special attention to self-esteem, lifestyle choices and future goals. The use of art, creative writing, and dialog in this group uniquely honors each woman's struggles and triumphs. Two eight-week sessions are available. 2005 schedule: Session B: April 26 - June 14, 2005; Session C June 21 - August 9; Session D September 13 - November 1. The cost is $40 per group for 8 weeks. The group will benefit women, men and teenagers struggling with bulimia, anorexia, binge eating disorder, and/or compulsive overeating (minimum of 14 years of age).
