Traumatic Brain Injury Targeted
  Family Support Program
The Tennessee Department of Health has partnered with The Arc Davidson County & Greater Nashville to administer the Traumatic Brain Injury (TBI) Targeted Family Support Program effective July 1, 2017. The intent of the statewide program is to benefit the individual with disability due to traumatic brain injury. This program will assist those individuals and their families to remain together in their homes and community. The program is modeled after the DIDD Family Support Program as described in the Family Support (TCA 33-5-201). The statewide program will enroll 119 individuals and grant a $2000.00 allocation per fiscal year. Persons currently enrolled in the ECF and CHOICES waivers are eligible to apply. One cannot be enrolled on both the Tennessee Family Support Program and the Traumatic Brain Injury (TBI) Targeted Family Support. If you or your family member have a disability due to traumatic brain injury please contact Asia Corder, Traumatic Brain Injury Family Support Coordinator at accorder@arcdc.org or 615-321-5699 x 229.

THE ARC'S FAMILY SUPPORT GRANT PROGRAM
In May 1992, the Tennessee General Assembly passed legislation establishing a statewide family support program.  Statewide funding for this program is determined by our state legislators on a yearly basis.  The program is administered by the State of Tennessee Department of Developmental and Intellectual Disabilities and managed by community agencies.  In Williamson County, the Family Support Program is managed by The Arc Williamson County.  This program went into effect July 1, 1992 and is designed to provide financial assistance and community resource information to individuals with disabilities and their families. Persons with a severe disability which is likely to continue indefinitely and results in substantial functional limitations in 3 or more areas of major life activities are eligible for this program. Individuals whose primary disability is a mental health illness are not eligible.  Persons must be a full time resident of Tennessee; be a U.S. citizen; and live in the community in an unsupported setting.  Individuals living in group homes, foster homes or other residential settings are not eligible for services. Individuals who receive services through a Medicaid waiver are not eligible. Services are designed to be flexible and responsive to family needs.  Services might include reimbursement for: respite care, day care, equipment for home, automobile modifications, specialized supplies, specialized equipment, health related, and summer camp.  Information about community resources is also available. Applications are accepted anytime throughout the year, but if you are determined eligible and all funds have been allocated, you will be placed on a waiting list. Remember that along with the application, we need proofs of residency, citizenship and disability. Refer to the Family Support Handbook for descriptions of acceptable forms of proofs.   CLICK HERE for the 2017-2018 application.  If you have any questions, please call Sharon Bottorff at 615-790-5815, ext 3 or e-mail at sbbarc@thearcwc.org.
