Through generous funding by the
Ralph C. Wilson, Jr. Foundation to our Caregiver Navigation program we were able to engage strategic planners to help us better identify the needs of caregivers in Allegany, Cattaraugus and Chautauqua counties. The first step of the plan is a community survey.
If you have now, ever have, or anticipate taking care of one or more loved ones (family members, friend or neighbor), I invite you to take our survey to help us to better support you (and others like you) in our community. The information will be used to determine what resources and services are needed in the community. The individual response will be kept confidential and will be combined with everyone who completes the survey.
Additionally, if you know of other people who support loved ones, please feel free to share this survey with them.

The delivery of health services, both physical and behavioral health, has been rapidly expanding to telehealth and telemental health services over the past six months. Cattaraugus County Health Department would like to hear from our community regarding their experiences with telehealth. They know that some people have things that prevent them from reaching out for services as well, and we would like to know if the opportunity for telehealth would allow them to overcome those things that prevent them from reaching out to us. They have developed a quick 8 question survey to poll public opinion. Please pass this survey on to as many people as you feel appropriate.
